Chronic Pain

Pain that is persistent, often lasting more than six months. Chronic pain can have numerous causes diseases such as Rheumatoid arthritis, osteoarthritis and fibromyalgia are well-known culprits. Persistent pain may also be due to such ailments as cancer, multiple sclerosis, stomach ulcers, AIDS, and gallbladder disease.

Muscle Spasms

A sudden involuntary contraction of a muscle or group of muscles. A sudden, transitory constriction of a passage, canal, or orifice; spasms usually occur when the nerves supplying muscles are irritated, and are often accompanied by pain.

Severe Weight Loss

Reduction of the total body mass, due to a mean loss of fluid, body fat or adipose tissue and/or lean mass. It can occur unintentionally due to an underlying disease or can arise from a conscious effort to improve an actual or perceived overweight or obese state.


An anxiety disorder caused by exposure to an intensely traumatic event, such as rape or assault, military combat or bombing of civilians, torture, death camps, natural disasters, terrible accidents, developmentally inappropriate sexual experiences, or life-threatening illness.


A group of eye diseases characterized by damage to the optic nerve usually due to excessively high intraocular pressure (IOP). This increased pressure within the eye, if untreated can lead to optic nerve damage resulting in progressive, permanent vision loss.


A sudden attack, spasm, or convulsion, as in epilepsy or another disorder. Symptoms may stop after a few seconds or minutes, or continue for up to 15 minutes. They rarely continue longer. Seizures of all types are caused by disorganized and sudden electrical activity in the brain.

Severe Nausea

A consistent sickness at the stomach, especially when accompanied by a loathing for food and an involuntary impulse to vomit. Nausea is a common symptom that may accompany many diseases and conditions.

Other Conditions

Any condition that is severe for which other medical treatments have been ineffective and the symptoms can be expected to be relieved by the medical use of cannabis.

    I am a MMCC patient and had my initial evaluation in October 2018. I am now required to have a one year follow up appointment but the office that I used last year seems to no longer be in business. Does your office accommodate follow up appointments? I can provide the necessary medical documentation and such.
